Travel With Confidence and Proper Permission

A travel authorization, sometimes called a minor travel consent letter, documents a parent’s or legal guardian’s permission for a child to travel without one or both parents.

A travel authorization may be needed when:

· A child is traveling with only one parent

· A child is traveling with a grandparent, relative, family friend, teacher, coach, or another authorized adult

· A child is traveling through an airline’s unaccompanied-minor service

· A child is traveling on a school, church, sports, or group trip

· A child is traveling outside the United States

· A parent, airline, cruise line, border official, or foreign authority requests proof of parental permission

The document helps establish that the child’s travel has been authorized and can help address concerns involving parental consent, custody, or the possible unlawful removal of a child.

What Information Is Included?

Depending on the circumstances, a travel authorization may include:

· The child’s complete legal name and date of birth

· The child’s passport or identification information

· The names and contact information of the parents or legal guardians

· The name of the accompanying adult or responsible traveler

· The accompanying adult’s passport or identification information

· The destination and purpose of the trip

· Departure and return dates

· Airline, cruise line, flight, or itinerary information

· Authorization for emergency medical care

· Emergency contact information

· The consenting parent’s or guardian’s signature

· A notarial certificate

Document-Preparation Fees

GVA Services Multiples LLC prepares travel authorizations based on the information provided by the parent or legal guardian.

Travel-authorization preparation generally costs $30 to $50, depending on the length and complexity of the document, the number of children or travelers, the destination, and the amount of travel information that must be included.

If you already have a completed travel authorization and only need the signature notarized, the notarization fee is $10.

Notarization Requirements

The person whose signature will be notarized must:

· Appear personally before the notary

· Sign in the notary’s presence when required

· Present a valid and acceptable form of identification

· Understand the document

· Sign willingly and without coercion

Please do not sign the document before your appointment unless instructed otherwise.

What to Bring

To help us prepare your travel authorization, please provide:

· Valid identification for the consenting parent or legal guardian

· The child’s full legal name and date of birth

· A copy or photograph of the child’s passport, if applicable

· The accompanying traveler’s complete information and identification

· Travel dates and destinations

· Flight, cruise, or other transportation information

· Hotel or lodging information, when available

· Emergency contact information

· Any special instructions or medical authorization requested

· Any existing parenting plan, custody order, or court order that affects the child’s travel

Important Information

Requirements vary depending on the airline, cruise line, destination country, custody arrangement, and circumstances of the trip.

A notarized travel authorization does not replace a passport, visa, court order, or any other travel document required by an airline or government authority.

Parents and guardians are responsible for confirming the current requirements of the airline, cruise line, destination country, consulate, embassy, and appropriate government agencies before traveling.

If an existing court order restricts travel or requires court approval, a privately prepared travel authorization may not be sufficient.
