Guardianship and Guardian Advocacy Form Preparation

Osceola County, Florida Only

Guardianship is a court-supervised process through which a judge may appoint a qualified person to exercise certain decision-making rights for someone who is unable to exercise those rights independently.


A guardianship proceeding may involve an adult affected by Alzheimer’s disease, dementia, a serious cognitive impairment, or another condition affecting the person’s ability to understand information and make informed decisions.


Guardian advocacy is a related but different process that may be available for a person with a qualifying developmental disability, such as an intellectual disability, autism, cerebral palsy, spina bifida, or another condition recognized under Florida law.


The court not the petitioner or document preparer decides whether an appointment is necessary, who will be appointed, and which specific rights and responsibilities will be granted.

Our Form-Preparation Service

GVA Services Multiples LLC provides nonlawyer guardianship and guardian-advocacy form preparation exclusively for cases filed in Osceola County, Florida.


Our service is limited to typing information provided by the customer into the appropriate courthouse packet. We do not decide which proceeding you should file, select legal rights for you, prepare legal arguments, or advise you about what information you should include.


The fee for completing the packet is $100. This price applies to the clerical preparation of the required forms, including applicable general guardian or standby guardian sections contained in the packet provided by the Osceola County Clerk.


Court costs, filing fees, background checks, fingerprinting, required education, medical reports, attorney fees, service fees, and other outside expenses are not included.

Obtain the Correct Packet First

Before coming to our office, you must contact or visit the Osceola County Clerk of the Circuit Court’s Guardianship Division and obtain the correct packet for your circumstances.

You are responsible for confirming the correct department, floor, packet, filing fee, and current filing requirements directly with the Clerk’s Office.


After obtaining the packet, bring the complete packet to GVA Services Multiples LLC with all requested information.

Information You May Need to Provide

Depending on the packet and type of proceeding, we may request:


  • A valid government-issued identification card
  • Social Security number or other information requested  by the court forms
  • Complete legal names, addresses, and dates of birth
  • Residential history
  • Employment and work history
  • School and education history
  • Information about the person who may need assistance
  • Information concerning the person’s disability or condition
  • Names and addresses of relatives or other interested  persons
  • Medical, educational, or supporting records requested  by the packet
  • Information regarding a proposed standby guardian
  • Complete answers to every question contained in the courthouse packet

We only enter information provided by the customer. We do not investigate, verify, or independently obtain missing information.

Completion Time

Document preparation generally takes approximately one week after we receive:


  1. The correct and complete courthouse packet;
  2. All required personal and supporting information; and
  3. Payment for the form-preparation service.


Incomplete information may delay the preparation of the documents.

Reviewing and Signing the Documents

After the forms are prepared, the customer must return to review the entire packet for accuracy and completeness.

The customer is responsible for:


  • Reading every completed form
  • Confirming that all information is accurate
  • Correcting any errors or missing information
  • Signing the documents in the proper locations
  • Obtaining notarization when required
  • Filing the packet with the Osceola County Clerk
  • Paying all court and outside fees
  • Attending all required hearings
  • Completing required background checks, fingerprinting, or training
  • Following all instructions, deadlines, and court orders


We will provide the customer with the completed paper packet and an available digital copy of the documents prepared by our office.

Missing or Withheld Information

Every required question must be answered before we can complete the packet. If the customer cannot or does not wish to provide required information, we may be unable to finish the forms.


In that situation, we will return the packet with the information already entered. The customer may complete the remaining portions independently or consult a licensed Florida attorney.


Payment covers the document-preparation work performed and is not conditioned upon the court accepting the filing or granting the requested appointment.

Attorney Requirements

Some Florida guardianship proceedings require representation by a licensed Florida attorney. Guardian-advocacy cases may have different representation requirements, but an attorney may still be required by law or ordered by the court, particularly when authority over property is requested.


Our $100 document-preparation service does not include an attorney and does not replace any attorney required by law or by the court.


The person who is the subject of a guardian-advocacy petition has separate legal rights, including the right to counsel. The court appoints counsel for that person as required by Florida law.

Important Limitations

GVA Services Multiples LLC:


  • Does not determine whether guardianship is necessary
  • Does not diagnose incapacity or a developmental disability
  • Does not promise that a guardian will be appointed
  • Does not choose which rights should be requested or delegated
  • Does not represent customers in court
  • Does not communicate with the judge on a customer’s behalf
  • Does not monitor the case after the documents are returned
  • Does not complete annual reports or continuing guardianship obligations unless separately agreed
  • Does not guarantee that the Clerk or court will accept the documents
  • Does not provide legal advice


The judge has exclusive authority to determine whether guardianship or guardian advocacy is appropriate and what powers, duties, and limitations will apply.

Nonlawyer Disclosure

GVA Services Multiples LLC is not a law firm. Vanessa is a Florida notary and nonlawyer document preparer. She is not an attorney and cannot provide legal advice, select legal remedies, interpret court rules, or represent anyone in a guardianship proceeding.


Our role ends when the forms have been completed using the customer’s information, reviewed and signed as required, and returned to the customer. All remaining requirements are handled by the customer, the Clerk of Court, the appointed attorneys, and the court.
